Homebased Therapist

Primary Functions:

  1. Recognizes and commits to critical importance of customer satisfaction (both internal and external customers).
  2. Provides comprehensive bio-psychosocial assessments in the home, school, community, or work site.
  3. Develops individualized treatment plans with a focus on improving individual and family functioning in the home, school, workplace or community.
  4. Provides effective and efficient mental health interventions including, but not limited to individual, family and process group therapies and crisis intervention appropriate to client needs. Interventions will primarily be provided in a home, school, community-based treatment setting, or work site.
  5. Collaborates with multi-disciplinary treatment team in planning, facilitating, and coordinating treatment especially in the areas of assessment, referral, and discharge planning appropriate to client needs.
  6. Identifies needs and provides links through case management to other treatment programs, community resources, services, and entitlements appropriate to client needs.
  7. Develops and implements effective and efficient treatment discharge plans as is appropriate to client needs.
  8. Actively involved in service line quality improvement efforts.
  9. Actively involved in professional development and education.
  10. Trains and assists clients and/or family in the development of activities of daily living, individually or in groups with a focus on improving client and/or family’s ability to function independently in the community.
  11. Provides transportation of clients in personal vehicle and/or company van as needed for service provision.
  12. Provides psychological first aid in response to a disaster or critical incident, if trained to do so.
  13. Complies with all applicable Behavioral Health Services policies and procedures.
  14. Participate in Aggressive Behavior Management appropriate to the population served. See Behavioral Health Services Policy: Provision of Aggressive Behavior Management Training of Staff.
  15. Individuals may not perform all of these duties, or may perform additional related duties not listed here.

Skills & Experience:

  • Master’s Degree from an accredited college or university in a mental health related field. (Required)
  • Two (2) years’ experience providing mental health treatment (Required)
  • Required Certifications/Licensures: (Valid Indiana license (LCSW, LMHC, LMFT) by Health Professions Bureau, Current Indiana Driver's License)
  • Ability to independently manage time and priorities
  • Ability to independently resolve problems
  • Ability to travel between locations
  • Ability to use patient care skills to provide an exceptional experience
  • Must have interpersonal skills including motivation, flexibility, persistence, and patience
  • Must have the ability to operate a motor vehicle in accordance with LCFS policy (Current Indiana Driver's License & Full Coverage Insurance)
  • Must have the ability to relate in a non-judgmental and culturally competent manner
  • Must have the knowledge of behavioral health competencies specific to population being served
  • Must possess effective organizational skills
  • Must possess knowledge and creativity in regard to community resources
  • Must possess knowledge of or readiness to learn strengths oriented Recovery and Resiliency Models
  • Must value client/family centered care and able to apply strengths based needs assessment
  • Ability to communicate effectively both orally and in writing

Working Environment:

  • Able to work hours needed to accomplish assigned tasks, to include evenings, weekends, & holidays when necessary
  • Use of personal vehicle (mileage reimbursement) to travel to client homes & work with clients inside their residence.
