Home-Based Program Coordinator

Duties and Responsibilities:

  • Supervisory Functions: Lead the coordination and oversight of clinical services, including providing onsite supervision, ensuring staff compliance with standards, and addressing customer concerns. You will play a crucial role in achieving program targets and ensuring exceptional patient experiences.
  • Client/Group Services: Provide effective mental health interventions and crisis interventions in various settings, including homes, schools, and community-based sites. Collaborate with multidisciplinary teams to develop comprehensive treatment plans and discharge plans tailored to each client's needs.
  • Care Plan: Work closely with treatment teams to facilitate treatment planning and coordinate services. Conduct psychosocial assessments, develop individualized treatment plans, and link clients to community resources and services.
  • Department Accountability: Adhere to all organizational policies and procedures, including participation in aggressive behavior management training. Ensure compliance with behavioral health services policies and maintain accountability for departmental operations.

Qualifications:

  • Master’s Degree in a mental health-related field
  • Three years of relevant clinical experience in mental health/behavioral health
  • One year of administrative leadership experience
  • Valid Indiana license (LCSW, LMHC, LMFT) and current Indiana Driver's License
  • Strong interpersonal skills, ability to manage time and priorities independently, and effective communication skills

Working Environment:
